New Patient Forms

What to Expect on Your First Visit

Your first visit will consist of a new patient comprehensive examination, an initial diagnostic basic cleaning, and a full set of x-rays (if you do not have current x-rays from another provider). A full set of x-rays is generally taken every three to five years. They allow us to see what is hidden below the gum line. Many problems can be identified on x-rays before they have had a chance to start hurting or cause any noticeable symptoms. We can also see your jaw, sinuses, and the general condition of your gum tissue and bone.

At our office, we will never take unnecessary x-rays, but in order to do the thorough job that you deserve, we must have a current set of x-rays. You can request copies of your x-rays from your previous provider and bring them with you or have them mailed directly to us. If we do not have your x-rays in time for your appointment, we will need to either take new ones or re-schedule as Dr. Rayne is not able to perform a complete exam without these.

Your initial new patient appointment will take approximately two hours of your time.

The initial diagnostic basic cleaning includes a periodontal examination by our hygienist. This is because we have no way of knowing what type of cleaning you will need and what length of appointment you will need until the examination is complete. The incidence of gum and bone disease has increased in the past several years, and we must provide the appropriate care accordingly. Cleanings range from a basic cleaning, which is what most people are referring to when they say "cleaning," to Therapeutic Periodontal (gum and bone) Therapy which is a method used to treat and manage Periodontal Disease.

We do file PPO insurance plans as a courtesy to our patients however we do so as an out-of-network provider. We will estimate your out of pocket costs based on information provided by your insurance company, and will expect payment of your portion of these charges on the day each service is provided. You are responsible for any amount not paid by your insurance.

For your convenience we accept Visa, Master Card, American Express, and Discover.  We also offer financing through Care Credit, allowing up to 18 months without interest if you qualify.  Information about Care Credit can be obtained through our office or by visiting www.carecredit.com.

We ask that as a general rule you do not bring children with you to your visits, for their own safety and in order to allow us uninterrupted time to focus on your care.  If you are able to bring a friend or family member to watch your children during your appointment, that is appreciated. If you have special circumstances arise as they sometimes do, please call the office ahead of time to see if we can accommodate you.

If you must cancel an appointment we ask that you give at least 24 hours notice. This will allow us time to schedule someone else that can benefit from that treatment time.

Cell phones are a part of everyday life, but in our office we try to limit their presence.  We ask that you turn off your cell phone while in the office, or set it on the vibration mode.  If you absolutely must take a call, please step outside so as not to disturb our other patients.
